Step 1 – Commence by downloading the form here on our site. You can open it up on your computer to type it out or you can print it off and complete it in blue/black ink.
Step 2 – Read through the information in the top third of the form to ensure you’re comfortable and aware of what it is you’re signing up for. When ready, enter in your name and address in the first available, fillable field. Next, provide your phone number, employee ID, and your SSN.
Step 3 – Below the SNN field, you will see the option to choose between one of the following two options: have the “total net check amount” deposited, or set an amount of each paycheck. The bank name and address has been filled in for you but you’ll have to provide the TD bank routing transit number and account number. You must also indicate the type of account that will receive the funds. Finally, enter in the date, print the document, sign it, and hand it into your employer or payroll officer for processing.
